Section 1: Agreement for the Volunteer Position.

This is an application to volunteer with WQT MASJID for which there is no monetary compensation. I understand that if I am accepted as a volunteer with WQT MASJID, I am agreeing to:

• Fulfill the orientation and training requirements for the volunteer role; and

• Abide by the policies and procedures of WQT MASJID.

Section 2: Confidentiality Agreement

I acknowledge that in the course of my volunteer work with WQT MASJID, confidential and privileged information may be shared with me in writing, verbally, electronically or in any other medium. The information is given in confidence and trust. I agree that during the course of volunteering with WQT
MASJID, and after my departure, I will keep all such information strictly confidential and not disclose such information to anyone under any circumstances. I understand that a violation of these obligations may be a cause for termination of my services with WQT MASJID without notice.

Section 3: Liability Release

I acknowledge that WQT MASJID cannot be held responsible for the loss, damage or theft of any belongings brought to any WQT MASJID program. WQT MASJID cannot be held responsible for any and all claims for damages arising as a result of any accident or injury or otherwise sustained by participants arising from WQT MASJID activity or program. I also agree to sign the Volunteer Waiver & Release provided to me along with this application and to promptly return a signed copy to WQT MASJID.

Section 4: Administrative Requirements

At WQT Masjid, we route most of our communications via email, and phone number. As such, access to email and phone is required. Communications between volunteers are primarily routed through office email address- would be provided upon joining.

Section 5: Media Release

If selected to become a volunteer of WQT Masjid, I hereby grant permission to WQT Masjid to use and publish my name, photograph, image, video footage, and any other media content captured or recorded during my participation as a volunteer. I also understand and agree that this media content may be used for promotional, marketing, and communication purposes by WQT Masjid, including but not limited to the organization’s website, social media channels, newsletters, press releases, brochures, and other printed and digital materials. I acknowledge that WQT Masjid retains all rights to the media content and that I will not receive any compensation or financial remuneration for the use of the media content. I further release WQT Masjid, its representatives, employees, and volunteers from any claims, demands, or liabilities arising out of or in connection with the use of the media content, including any alterations or modifications made to the media content.
I have read this, understand its contents, and voluntarily agree to its terms. This consent is given in perpetuity, and does not require prior approval.
